Successful hoof rehabilitation is not about choosing a particular shoe, boot or trimming method.
The goal is to restore the hoof capsule to the best possible alignment with the internal structures of the foot, reduce damaging forces on weakened laminae, and allow a new, well connected hoof to grow.
This takes time, regular trimming and, ideally, radiographs to show exactly what is happening inside the foot.
• Laminitis weakens the connection between the hoof wall and the pedal bone.
• The external appearance of the hoof does not always tell us where the pedal bone is positioned inside it.
• Lateral radiographs are extremely useful for assessing alignment and guiding the trim.
• Where rotation, descent or hoof capsule distortion has occurred, a realigning trim is required.
• Long toe and flare create leverage on weakened laminae and need to be addressed.
• The aim is to establish appropriate breakover and restore the relationship between the hoof capsule and the internal structures.
• Support should be spread across appropriate structures of the foot rather than relying heavily on the damaged hoof wall.
• Frequent trimming is important. ECIR recommends approximately two weekly intervals during the early stages of rehabilitation.
• Boots, pads and conformable footing can provide support and comfort, but they cannot compensate for an incorrect trim.
• A complete new hoof capsule can take many months, sometimes up to a year, to grow.
Inside a healthy hoof, the hoof capsule and pedal bone have a very specific relationship.
Laminitis damages the laminae connecting the two. If that connection weakens sufficiently, the hoof capsule and pedal bone can lose their normal alignment.
This may appear as rotation, sinking or descent, a stretched white line, flare, a long toe, changes in sole shape or a visible change in the angle of the hoof wall.
Hoof rehabilitation is about restoring the hoof capsule around the internal structures as correctly as possible and maintaining that relationship while new hoof grows.
Looking at the outside of the hoof gives us useful information, but it cannot show us everything.

It is recommend lo have lateral radiographs after laminitis because they show the position of the pedal bone within the hoof capsule. This allows the veterinarian and hoof care professional to see whether rotation or descent has occurred and establish appropriate trimming goals.

This is particularly important when the hoof has become significantly distorted.
Radiographs and trimming really work together. The radiograph tells us where the internal structures are, and the trim works to bring the external hoof capsule back into the best possible relationship with them.
If the hoof capsule and pedal bone are no longer correctly aligned, ECIR describes the corrective approach as a realigning trim.
Two important goals are:
• Reducing abnormal forces that could continue damaging the laminae.
• Encouraging new hoof wall to grow with a healthier connection.
Exactly what needs to be done will depend on the individual foot and the radiographs. This is where an experienced hoof care professional becomes important.
The aim is not to make the hoof conform to a particular external shape. It is to trim according to the structures inside it.
Just to give you an idea of how it works, let’s look at a typical laminitic hoof and see what needs to be done to achieve re-alignment of the pedal bone.
In the picture below, we can see a typical laminitic hoof of a Shetland pony.

The second picture shows where the pedal bone might be expected to be sitting inside the hoof.

The main points to consider in this photo include:
• The degree of rotation of the pedal bone and hoof capsule, the hoof wall is not parallel with the front surface of the pedal bone.
• Break over is way out the front of the hoof which places more mechanical force on the laminar connection which is already compromised.
• The heels are very high, tipping the rear of the pedal bone up and the tip down towards the ground.
The red lines show the ultimate goal of the re-aligning trim. The objectives are to re-align the pedal bone with both the hoof capsule and the ground surface, this will be achieved by:
• Lowering the heels to bring the plantar (bottom) surface of the pedal bone to within 3-5° of the ground plane
• Bringing break over point back in line with the dorsal wall of the pedal bone
• Balancing the hoof so that the new hoof growth is allowed to grow straight and parallel with the pedal bone (provided the cause of the laminitis is addressed)
The new hoof shape indicated by the red lines will take some time to grow in. Full realignment may take 7-8 months depending on the horse and management. The yellow lines indicate what we may be able to achieve on the first trim.
The last photo shows the hoof after trimming… not quite there, but getting close!

With a very sore horse it may be necessary to do the first trim in stages. The horse may have great difficulty lifting it’s hooves off the ground to be trimmed. Using foam pads under the supporting leg may help or it may be necessary to trim the horse when its lying down.
A long or flared toe creates leverage as the horse breaks over the front of the hoof.
When the laminae are already weakened, that leverage places additional stress on the connection between the hoof wall and pedal bone.
One of the fundamental principles is therefore bringing the toe and breakover back into an appropriate relationship with the current position of the pedal bone.
This does not mean simply rasping the front wall thin to make the hoof look straight.
The goal is mechanical realignment, not cosmetic improvement.

Another important principle is reducing excessive reliance on the damaged hoof wall for support.
It's important to create a broader base of support involving the frog, caudal structures and solar surface of the foot. Spreading the load over a larger appropriate surface area reduces pressure on the weakened laminar connection.
Conformable footing and appropriately fitted pads can help support this process while the foot heals.
One correct trim is not the end of hoof rehabilitation.
The hoof continues to grow, and if trimming intervals are too long, the toe and hoof capsule can gradually move away from the desired alignment again.
Two weekly trimming intervals are recommended during the initial stages of rehabilitation.
The amount removed at each trim may be small. The purpose of frequent attention is to maintain the alignment that has been established while the new hoof grows down.
Intervals can be reassessed as the hoof becomes more stable.
There are many products and techniques marketed for laminitic horses.
However the answer is refreshingly straightforward: no device can compensate for an incorrect trim.
Boots and pads are often useful because they provide protection and allow frequent access to the hoof for trimming. ECIR's 2025 summary describes its core approach as keeping the horse barefoot in boots and pads so the feet remain readily accessible.
However, ECIR also acknowledges that some horses may benefit from shoes, casts, glue on appliances or other forms of support depending on the individual situation.
So this does not need to become a barefoot versus shod argument.
The important question is:
Does the approach support correct mechanics and allow the hoof to rehabilitate?
Once the underlying trigger is controlled and hoof mechanics improve, new hoof growth from the coronary band can begin to tell the story of recovery.
You may see new wall growing at a different angle from the older damaged hoof below it. As this healthier growth progresses towards the ground, flare and distortion can gradually be removed.
Other improvements may include:
• A tighter white line
• Less flare and distortion
• Better hoof proportions
• Improving sole shape and depth
• Increasing comfort
Regular photographs taken from consistent angles can make these changes much easier to follow.
It takes time and the realigning trim is the beginning, not the end. It can take up to a year for the hoof capsule to completely grow out and be replaced, therefore requires patience.
We are not trying to make the old damaged hoof look perfect, we are trying to maintain the correct mechanical environment long enough for the horse to grow a better one.
Hoof rehabilitation after laminitis is fundamentally about alignment and time.
Find out where the internal structures are.
Trim the hoof according to those structures.
Reduce damaging leverage on weakened laminae.
Maintain the alignment with frequent trimming.
Provide appropriate support and comfortable footing.
Then allow the new hoof to grow.
There is no particular shoe, boot or trimming system that can replace those basic principles.
A new hoof capsule that is correctly aligned with the internal structures and has a strong, healthy laminar connection.
